• Application on specific Performa (to be obtained from respective ZSB)
• Photostat attested copy of Discharge Book/Certificate
• Pension Certificate issued by PDA
• Age proof of Daughter (Birth certificate/school leaving cert)
• Marriage Registration Certificate
• Bank Account details of ESM
